Fees and Insurance
Most insurance companies cover the treatment of symptomatic varicose and spider veins. At the time of your consultation, our office staff will explain the costs that you can expect for treatment and provide you with information concerning the insurance provisions of your individual plan. For your added convenience, we will file the paperwork for you. We participate in most insurance plans.

All asymptomatic patients are required to pay personally to this office, unless they have insurance that covers cosmetic therapy. |

All symptomatic patients are billed to the insurance company. Symptoms include one or more of the following: Pain, Cramping, Itching, Burning, Swelling, and Bleeding, Ulcerations, and Skin Discoloration. Patients may also have conditions such as: Blood Clots, Superficial Thrombophlebitis, Eczema and long saphenous vein incompetency. |

For HMO patients, referrals are required from the primary care physician before the patients are seen in the office. Prior to starting any treatment, a referral from the primary care physician and approval is obtained from the insurance company. Any required information requested by the insurance company is immediately faxed or mailed to them. Upon the approval from the insurance company the patient is notified and the necessary treatments are started. |

Treatment is only started after the authorization is obtained from the insurance company and is noted in the patient's chart. In the case of a denial, an appeal process is started. The treatment only starts after the approval. If the insurance company continues to deny the treatment, the patient is then notified and if he/she requests we will start the treatment. However, the patient is fully responsible for the cost, which is discussed before any treatment is done in detail. |

Patients are required to have complete pictures taken of all of the aspects of the legs and lesions. However, they are responsible for payment of these pictures. |
